All the forms necessary for persons in process to become an Associate Member of the New England Conference. This includes: 1. A basic overview sheet, a letter of instruction, and a Checklist of Requirements. This will give you further details on all of the materials that are necessary for submission. 2. An Application for Clergy Relationship 3. A Personal and Professional Information Sheet 4. A Medical Report Form 5. Form 112 - Request for Psychological Assessment materials 6. Confidential Lay/Clergy Reference Forms (you will need 5 copies of the lay form, and 2 copies of the clergy form) 7. Appendix A: A Definition of Transformational Pastoral Leadership (5 copies needed - send with each lay reference form) 8. A Policy for the Candidacy Examination Process 9.
