Annual Wellness Exam Verification Form (pdf)

All Active Health Insurance Participants and their Spouses who have a Wellness Examination (Physical) in 2008 and submit this signed verification form from your Physician will receive $100(each). Have your Physician sign the Incentive Verification form upon completion of the Annual Wellness Examination (Physical), mail it to the address on the form.
